New Patients

We are accepting new patients for both primary care and rheumatology. The forms to register your child(ren) and obtain records from the previous primary are located here. For primary care patients we would like records from the previous primary care provider before scheduling however, we do understand that a sick child will take priority. We will not schedule a well visit until we can obtain records and review them.  For Rheumatology patients please see our procedures here.


Office Visits

We ask that when appointments are made that you keep track of the date and time. We do provide a curtesy reminder service by both texting and/or calling. We ask that you listen to the entire message before calling the office. Please provide at least 24 hours’ notice for any cancellation or re-schedule. Failure to do so will cause a $25.00 fee to be charged. If you are more than 10 minutes late we reserve the right to reschedule your appointment. We ask that you arrive at least 10 minutes early for your appointment and to bring your insurance card. Your insurance company requires us to collect your co-pay prior to your visit so we ask that you be prepared and if sending your child with someone else that they be prepared. Any outstanding balance will require at least a $25.00 payment at the time of visit. We expect all outstanding balances to be paid within 30 days. A return check will result in a charge of a $35.00 processing fee.


Lab Work and Results

Laboratory tests are often required by a provider for diagnosing your child. Please check with your health insurance company as the may have a preferred lab provider. Otherwise you are free to go to any lab. We can directly upload a script to Quest Diagnostics or you can ask us to fax the script to your favorite lab. Urgent or abnormal results will be called in to you as soon as they are reviewed by the provider. Any normal results will be discussed at your next visit. Please do not call the office and request that these be reviewed over the phone.


After Normal Office Hours

By calling our main phone number, (716) 646-5188 after normal business hours you will be able to request a refill on your children’s medication, leave a message regarding an appointment or leave a message for a nurse regarding your child(ren).  If you have a situation that you feel needs to be discussed with a medical professional, the last option will connect you with our answering service. Leave the information with the service and within an hour you will receive a return call. We ask that you stay by you phone and answer promptly, also please unblock your phone to receive our call if needed.


Forms and Paperwork

At your child’s annual well visit we will provide you with a Health Assessment Form and an up to date immunization record. Any additional paperwork required by you should be brought to that appointment (if possible) and with your part of the form COMPLETELY filled out. Please let us know if we are to mail the forms to you (provide a stamped self-addressed envelope), fax to a number you provide or you will pick up the completed forms. Please allow up to 7 to 10 DAYS FOR COMPLETION (additional time if mailed).
